We make every effort to keep down the cost of your medical care.
We are members of most insurance plans, such as Blue Cross, Blue Shield, United Healthcare, Healthnet, Aetna, and Cigna. We are also a member of Brown and Toland Medical Group and accept Medicare. We DO NOT accept MediCal.
In most cases, if we are a preferred provider on your insurance plan, and if the service provided by our office is covered, we will accept your insurance payment. You will be responsible for ALL co-pays, deductibles, and non-covered services. We accept all major credit cards, personal checks and cash.
Our billing office will first submit a claim to your insurance carrier(s). When the claim is processed, your insurance will also send an Explanation of Benefits (EOB) to you.
PLEASE NOTE: With the continual change in insurance coverage, it is a good idea to check if we are a participating provider on your insurance plan before your visit.
On your first visit or if you have changed insurances since your last visit, we ask that you provide us with all needed insurance information. We will make a copy of the front and back of your insurance card. This will help make billing run smoothly.
There will usually be a co-pay due at the time of your visit.
Most insurance plans have a deductible. If this has not been fully met, you will be responsible for the balance of the deductible not met.
If you have no insurance, we would appreciate payment at the time of service. If your situation prevents you from paying your bill promptly, arrangements can be made.
If you choose to do your own billing, a super bill or receipt will be supplied to you upon payment.
